Fitness Coaching Agreement

Effective Date: March, 2026

This Fitness Coaching Agreement (“Agreement”) is between Master Self Reliance, LLC (“Company,” “Coach,” “we,” or “us”) and the purchasing client (“Client,” “you”).

By purchasing, scheduling, or participating in any services provided by Company, the Client confirms they have read, understood, and agreed to this Agreement. Payment for services constitutes acceptance of these terms, whether or not this document is signed.


1. Services

Self Reliance provides fitness coaching services which may include workout programming, lifestyle and habit coaching, accountability support, educational resources, and virtual or in-person sessions.

All services are provided for educational and coaching purposes only and are not medical advice, medical treatment, physical therapy, or mental health care. 


2. Eligibility

By purchasing services, the Client represents that they:

are at least 18 years old

are legally able to enter a binding agreement

have provided accurate and truthful information

If services are provided to a minor, a parent or legal guardian must consent.


3. Payment and Acceptance

Payment for services, and any use thereof, constitutes acceptance of this Agreement.

All fees must be paid as stated at purchase. The Company may suspend or terminate services if payment obligations are not met. Payment plans must be completed according to their agreed schedule.

No signature is required for this Agreement to be enforceable once payment has been made or services have been rendered, partial or full.


4. No Refund Policy

All sales are final unless otherwise stated in writing by Master Self Reliance, LLC, Kathryn Sleboda or authorized employees.

Due to the nature of coaching services, time reservations, and custom program development, no refunds will be issued for purchased sessions, packages, programs, or materials.

This applies regardless of scheduling conflicts, change of mind, lack of results, non-participation, relocation, or termination resulting from violation of this Agreement.


5. Cancellation and Scheduling

Clients must provide at least 24 hours written notice to Master Self Reliance, LLC or applicable coach/representative in order to cancel or reschedule a session and avoid the loss-of-session penalty. 

Sessions canceled with less than 24 hours notice, missed sessions, or late arrivals will be forfeited at the Company’s discretion.

The Company may reschedule sessions when necessary and will make reasonable efforts to provide an alternative time.


6. Client Responsibility and Results Disclaimer

Fitness results vary based on many factors including consistency, nutrition, sleep, lifestyle, health status, and personal effort.

The Company makes no guarantees regarding results including weight loss, muscle gain, strength improvements, or performance outcomes.

The Client accepts full responsibility for their participation, decisions, and results.


7. Health Disclaimer and Medical Clearance

Participation in exercise and fitness activities involves inherent risks including injury or illness.

The Client assumes full responsibility for participation and agrees to consult a physician before beginning or modifying any exercise or wellness program.

The Client agrees to notify the Company of any injuries, medical conditions, or limitations that may affect participation.

The Company does not diagnose, treat, or provide medical advice.

The Company and its employees/representatives are not doctors and cannot provide medical advice. Any suggestions regarding changes to The Client’s exercise plans, diets, meal plans, macronutrient intake, supplements, hormones, and peptide usage are for informational and educational purposes only. All suggestions must be presented to The Client’s doctors or chosen medical professional before implementation. The Company and its employees, coaches, and representatives assume no liability for The Client’s changes in exercise, diet, supplement, hormone/peptide usage based on information, suggestions or opinions expressed during coaching.


8. Assumption of Risk and Liability Waiver

To the fullest extent permitted by law, the Client assumes all risks associated with participation in coaching services and releases Self Reliance Performance Coaching and its owners, employees, and affiliates from liability for injuries, damages, or losses arising from participation.

The Company shall not be liable for indirect, incidental, or consequential damages related to the services. 


9. Client Conduct

Master Self Reliance, LLC maintains a professional and respectful working environment.

Clients must communicate respectfully and maintain appropriate professional behavior. The following conduct is prohibited (including but not limited to):

harassment or abusive language

sexual harassment or sexually inappropriate communication

threats, intimidation, or discrimination

repeated disrespectful or hostile communication

excessive messaging or boundary violations

The Company has zero tolerance for abusive or inappropriate behavior. 

Services may be immediately suspended or terminated without refund if the Client engages in such conduct. Whether or not behavior and/or communication is deemed inappropriate shall be determined by The Company and/or Coach/Representative, not the client. 


10. Termination of Services

The Company may refuse, suspend, or terminate services if a Client:

violates this Agreement

fails to make payments

behaves inappropriately or unprofessionally

shares proprietary materials without permission

interferes with the coaching relationship

Termination does not eliminate any outstanding payment obligations. 


11. Communication Policy

Communication is limited to the platforms and frequency included in the purchased service.

Responses are not guaranteed outside business hours. Excessive messaging or demands for immediate responses may be considered a violation of this Agreement.


12. Intellectual Property

All coaching materials, programs, videos, documents, templates, and resources remain the intellectual property of Master Self Reliance, LLC.

Materials are for personal use only and may not be copied, shared, reproduced, distributed, or sold without written permission.

Sharing passwords, links, recordings, or coaching materials with others is prohibited and may result in termination of services without refund. 


13. Non-Transferability

Coaching services are personal to the Client and may not be transferred, shared, or used by another individual without written approval.


14. Electronic Communication

By working with the Company, the Client agrees to receive electronic communications including scheduling messages, invoices, and service updates.

Clients are responsible for maintaining current contact information.


15. Indemnification

The Client agrees to indemnify and hold harmless Master Self Reliance, LLC and its representatives from any claims, damages, liabilities, or expenses arising from participation in services or violation of this Agreement.


16. Governing Law

This Agreement is governed by the laws of the State of Florida.

Any dispute arising from this Agreement shall be resolved exclusively in Florida, unless otherwise required by law.


17. Entire Agreement

This document represents the entire agreement between the Client and Master Self Reliance, LLC, and supersedes all prior discussions or agreements.


18. Severability

If any provision of this Agreement is deemed invalid or unenforceable, the remaining provisions shall remain in effect.


19. Modifications

The Company reserves the right to update this Agreement as is necessary and according to standard business practices. The version in effect at the time of purchase will apply to that purchase.


20. Acknowledgment

By purchasing, scheduling, or participating in services, the Client acknowledges that they have read and agreed to this Agreement.


 

21. Coaching Philosophy and Program Duration

Master Self Reliance is founded on the principle of developing long-term independence and self-sufficiency in its clients. At the beginning of a coaching relationship, clients typically require more frequent guidance, structure, and accountability. As the client develops the skills, habits, and systems taught through coaching, the level of support is expected to decrease as the client becomes more self-reliant.

For this reason, coaching programs are designed to gradually reduce the frequency of sessions over time. Programs often begin with more frequent meetings (such as weekly sessions), then transition to less frequent check-ins (such as biweekly sessions, monthly sessions, or periodic strategic check-ins).

Unless otherwise stated in writing, no coaching package extends beyond twelve (12) months from the start date. Coaching services are intended to support the client in developing the skills necessary to operate independently, and therefore, the frequency of coaching sessions typically decreases over the course of the coaching relationship until the program concludes.
